Periodic Health Assessment (PHA) is a yearly requirement that has replaced the physical exam requirement of every 5 years over the age 30. The PHA will still be a two part process completed at the Physical Exam Section (PES) of the Reception Medical Clinic.
Part 1: Before scheduling an appointment at PES, Login into AKO, click drop-down Self Service tab, Click My Medical Readiness, click highlighted link (Periodic Health Assessment), then complete PHA form. If computer access isn't available please let PES know prior to appointment or show up to scheduled appointment 45 minutes prior to access one of the computers at the PES.
Over 40 PHA: If you are over the age of 40, you will need to complete a blood draw and have a normal electrocardiogram (EKG) within the last 5 years. Blood draws will be scheduled for Service member when calling to make your appointment for your PHA and be done at Moncrief on the 3rd floor prior to coming to scheduled appointment. If Service Members EKG was done somewhere other than Fort Jackson a printout will be required or, Service Member can pick up an EKG sheet from BLDG 1890 and have an EKG done at the McWethy TMC Located Bldg. 4575, between Moncrief and Caldwell Dental Clinic.
Over 50 PHA: If you are over the age of 50, you will need to complete everything listed above for over 40 PHA along with a Hemoccult. Supplies for a Hemoccult can be supplied to Service Member as well at BLDG 1890.
Part 2: Show up to PES Section 10 minutes prior to scheduled appointment in PT uniform or ACU/OCPs as long as all uniforms are in accordance with uniform standard AR 670-1.
Disclaimer: If you are late to your scheduled appointment by 10 or more minutes, you will be placed on “Stand-by “status, meaning that you will be seen when the provider can fit you in. If you have not completed requirements for part 1, you will be rescheduled for a later date. If part 1 is complete but Service Member is unable to save the PHA form and continues to say “Incomplete” you will still be seen for part 2.
